* I hereby declare that I have not / will not visit any county that is defined as 'Red' in the 14 days prior to my arrival to the state of Israel.

A stay of up to 12 hours in an airline terminal of a country considered 'Red', without leaving the premises of the terminal, is not considered a stay in a red country for the purposes of this application.

* I hereby declare that I understand that my entry to the State of Israel will be conditional upon presenting a medical insurance policy which includes COVID-19 treatment to the border control authorities upon my arrival.

Please note: A visitor who has visited any country that is defined as 'Red' in the 14 days prior to his/her arrival to the State of Israel WILL NOT BE PERMITED TO ENTER the country.
In addition, please note the daily changes in the countries' definitions published by the Israeli Ministry of Health. In case of a change from 'Green' to 'Red' between the date the request was submitted to the date of arrival, the entry approval will be considered as cancelled.
